FINDINGS AND ORDER
OF REVOCATION
The Superintendent of Insurance issued a Notice of Opportunity for Hearing to each of the individuals listed below. The Notice was served on each individual pursuant to section 119.07 of the Revised Code. More than thirty (30) days have elapsed from the date of service or from the last date of publication and each of the individuals listed below has not requested a hearing.
After reviewing the records in these cases, the Superintendent finds that:
Each of the individuals listed below is licensed in this state as an insurance agent.
Each of the individuals listed below failed to comply with the continuing education requirements of section 3905.481 of the Revised Code for the 2008/2009 compliance period.
IT IS THEREFORE ORDERED that pursuant to section 3905.482 of the Revised Code, the Ohio insurance license of each individual listed below be and hereby is revoked. The revocation shall be effective March 1, 2012.

The notice of appeal shall set forth the order appealed from and the grounds of the appeal.
