A resident of Poplar Street was transported to Memorial Hospital after making threats of self-harm Monday at 1:37 a.m.
Antonio Cobb, 35, of Toledo, was arrested on an outstanding warrant from the Ohio State Patrol Monday at 6:24 p.m.
Heather Reck, 36, of Collins Avenue, was arrested on an outstanding warrant out of Union County Monday at 1:04 p.m.
A resident of East Fifth Street reported identity theft Monday at 5:15 p.m.
A resident of Boerger Road reported theft of a wallet from a motor vehicle Monday at 9:42 p.m.

Deputies and units from the Liberty Township Fire Department and Allen Township Fire Department responded to the 20000 block of Lunda Road for an unresponsive male with CPR in progress Monday at 4:20 a.m. EMS took the male to Memorial Hospital.
A deputy was sent to a home in the 11000 block of U.S. 36 for an unruly juvenile refusing to go to school Monday at 7:16 a.m.
A deputy was sent to a home in the 20000 block of Westlake Lee Road for an unruly female juvenile refusing to go to school Monday at 8:24 a.m.
A deputy investigated the possible theft of a grave bench at a cemetery in the 20000 block of Middleburg Plain City Road Monday at 9:11 a.m.
A deputy investigated the theft of a utility trailer loaded with rolled pipe that was parked near the intersection of Northwest Parkway and Darby Pottersburg Road Monday at 10:55 a.m.
A deputy responded to the U.S. 33 exit ramp to U.S. 42 to investigate a property damage carsh involving a 2005 Volvo XC9 that struck the back of a 2003 Chevrolet Venture Monday at 11 a.m.
A deputy went to a business in the 7000 block of Montgomery Drive to investigate the theft of a generator Monday at 12:51 p.m.
While on patrol, a deputy stopped to help a disable motorist on Industrial Parkway near Hillcrest Drive Monday at 5:09 p.m. After pushing the vehicle out of the roadway, the vehicle occupants were found with drug paraphernalia. Caleb S. Kise, 23, and Mark A. Borham, 24, both of Marysville, were charged with possession of marijuana paraphernalia.
A deputy was sent to a residence in the 200 block of West State Street in Milford Center to investigate a trespassing complaint Monday at 8:14 p.m.
